Beyond the Benefits

My Wishes Planning Guide

You have completed your End of Life Preparedness Check. This free guide gives you one place to write down your care wishes, your decision maker, your final arrangements, and where your family can find what they will need.

How to use it

  1. Start with the area where your score was lowest. Each section lines up with one question from your Preparedness Check.
  2. Fill in what you know today and leave the rest for later. You can work in any order and at your own pace.
  3. Finish by choosing three next steps and giving each one a date.
  4. Keep your completed guide somewhere safe, and tell someone you trust where to find it.

The areas from your Preparedness Check

  • Care wishes
  • Decision maker
  • Advance directive
  • Final arrangements
  • Important information

When you want support

Some of these pages are easy to fill in. Others, like naming what matters most or starting the conversation with your family, are harder to do alone. As a hospice nurse and trained end of life doula, I offer guided planning to help you and your family work through these decisions with care.
